There is a fee to petition the court for a name change, as well as many reporting requirements to inform your creditors of the change. It may be done without an attorney but it may be helpful to consult with a lawyer who works in this area to make sure you're doing it according to the statute.

Power of Attorney for Children
- This form can be used to give another person the authority to care for your child in an emergency. It should be used if you will be leaving your child in the care of someone else for more than just a day, but may be useful to provide to a daycare or pre-school in the event of an emergency. You can limit what powers are given. Simply fill in the appropriate spaces and delete the part you don't need.

Be cautious, there are individuals who will attempt to exploit your child through various ways, even the Internet. These predators exert a tremendous amount of time, money, and energy into seducing their potential victims with attention, kindness, affection, and gifts. These individuals are aware of children/teen interests and empathize with their problems. If you are not spending time listening and talking with your child, someone else will.
