- This is a complete set of the Kansas Protection from Abuse forms in English, with a Spanish translation underneath. The form should be compeleted in English, as much as possible, as the Court personnel who must review the forms may not be able to understand information provided in Spanish.

Kansas Protection from Abuse (PFA) forms
- These forms are provided by Kansas courts for use in filing for a Protection from Abuse order in Kansas. This link will take you to a variety of forms used in this type of court action. After reviewing the other forms, you must complete the Petition for Protection Order. If there are children involved, you must complete the UCCJEA Affidavit. These forms should be taken to the Clerk of the District Court, located in the County Courthouse. Blank forms are available from the Clerk of the District Court. If you choose to print and complete these forms, please do so in ink and as neatly as possible.

Power of Attorney for Children
- This form can be used to give another person the authority to care for your child in an emergency. It should be used if you will be leaving your child in the care of someone else for more than just a day, but may be useful to provide to a daycare or pre-school in the event of an emergency. You can limit what powers are given. Simply fill in the appropriate spaces and delete the part you don't need.
