Power of Attorney for Healthcare
Use this document (fillable form attached below) to chose the person who will carry out your health care instructions in case you are not conscious or are unable to make those decisions. You do not have to chose a family member. If you do, make sure to give that person a notarized copy. Also share the document with other family members and give a copy to your doctor. History of the War on Poverty
  As you may know, Kansas Legal Services is part of the national legal aid movement that was born in the War on Poverty in the early 1960s.  KLS grew out of the movement that believed everyone deserved equal access to justice, without regard to their ability to pay for services. The National Legal Aid and Defender Association (NLADA) has a film about the history of civil legal aid in the United States on their website.

The Age Discrimination in Employment Act of 1967 (ADEA) protects individuals who are 40 years of age or older from employment discrimination based on age. The ADEA's protections apply to both employees and job applicants. Under the ADEA, it is unlawful to discriminate against a person because of his/her age with respect to any term, condition, or privilege of employment -- including, but not limited to, hiring, firing, promotion, layoff, compensation, benefits, job assignments, and training.  http://www.eeoc.gov/facts/age.html

You have the right to make decisions now regarding your own health care in the future.  What is Advance Care Planning? Advance Care Planning is a process for helping you understand possible future health care choices. Reflect on your own values and goals and discuss your choices with those persons closest to you. You may also put your wishes in writing in case you become unable to make your own decisions in the future.
